Case Management is a service that is available in conjunction with many of the other services offered through Bona Vista Programs. Case managers track and coordinate all of the services that an individual with developmental disabilities may be eligible for through their various funding sources. Together with the person being served and their support team, the case manager uses Person Centered Planning to develop an Individualized Support Plan (ISP) that outlines wants, needs and dreams in the form of outcomes for the person being served. Case Managers then utilize informal and formal supports and services in the community to help persons being served meet all their identified outcomes. These outcomes can be related to services being received, health and safety or any other accomplishment the person being served and their team feel is appropriate. Case managers are then responsible for monitoring the progress that the person being served is making towards these goals and ensures that service providers or responsible party listed on the plan, are assisting the person being served in reaching these goals in an appropriate manner.

The Case Manager is a vital resource to individuals who receive various services. The federal and state regulations change frequently, the case manager tracks these changes and are able to be a type of connection between the individual and the services they are in need of or are receiving. Overall, the main role of the Case Manager is to monitor all services being received, to ensure that the person receiving services is in a healthy and safe atmosphere and that they are receiving quality, inclusive activities that are centered on their wants, needs, desires and dreams.
Case Management:
· Oversees, coordinates and refers to all available resources. · Uses Person Centered Planning to develop an Individualized Support Plan (ISP). · Case Managers utilize informal and formal supports and services in the community to help the person being served meet all their identified needs. · Is experienced and knowledgeable in the services available and the eligibility requirements for those services. · Helps people become more independent and become more integrated in their communities.
