Community Connections

KokCC1Community Connections is a day program for adults with developmental disabilities. These individuals generally need intense supervision and are not currently at the level needed to work independently in the community. We currently serve 35 individuals in our Bona Vista at 1800 S. Plate, Kokomo location in Howard County, and 10 individuals in our Bona Vista at 105 S. Benton Street, Peru location in Miami County.
 
Activities are focused on personal goals and improving daily living skills. These include cooking, exercise, music appreciation, academics,
pre-vocational skills, crafts, and much more.
 
Opportunities are also provided to participate in the community as a whole. This opens the door to socialization, responsibility, and volunteering. Staff works one-on-one with individuals to help them establish these vital connections.
 
Community Connections functions cooperatively with licensed therapists, nurses, social workers, behavior consultants, as well as other stakeholders to assist individuals in reaching their highest potential.
 
Peru CC1To be eligible for Community Connections services, you must:
  • Live in Howard, Miami, or a surrounding county
  • Be 16 years of age or older and no longer attending public school
  • Have one or more severe disability
  • Be eligible for funding or be able to self-pay
  • Want to learn skills to become more independent
  • Try hard to meet self-set goals
To enroll, you may be recommended by Vocational Rehabilitation Services, the school system, or through a vocational evaluation. If you feel this program is a good fit for someone in your family, do not hesitate to call. 

Services are paid for by the Bureau of Developmental Disabilities Services - Division of Disability, Aging, and Rehabilitative Services, Medicaid Waiver, and self-pay.
