Community Connections Adult Day Program

 
 

    Community Connections is a day program for adults with developmental disabilities. The individuals participating in this program generally need intense supervision and are not currently at the level needed to work independently in the community.

    Activities are focused on the individual’s personal goals and improving daily living skills.  Opportunities are also provided for these individuals to participate in the community as a whole. Community participation opens the doors to many opportunities for socialization, responsibility, and volunteer work. Staff work one-on-one with individuals to help them establish connections within the community.

    Community Connections works cooperatively with licensed therapists, nurses, social workers, and a behavior coordinator to assist individuals in reaching their highest potential.

 

    Who is eligible for Community Connections services?

 A person who wants to be in Community Connections must:

 
  • Live in Howard County, Miami County or a surrounding county.

  • Be 16 years of age or older and no longer attending public school.

  • Have one or more severe disability.

  • Be eligible for funding or be able to self-pay.

  • Want to learn skills to become more independent.

  • Try hard to meet the goals individuals set for themselves.

    How does a person enroll in Community Connections?

    A person may be recommended by Vocational Rehabilitation Services, the school system, or through a vocational evaluation.  It is also possible that someone could just hear about CC and know someone that would benefit from the program.

 

    What does a person do in Community Connections?

    A person decides according to their likes and dislikes what individual goals to work on with the help of their families and caregivers.  Some examples include:  cooking, volunteering in the community, exercise, daily living skills, music appreciation, academics,  pre-vocational skills, crafts, and much more.

 

    How are services paid for in Community Connections?

    Services are paid for by the Bureau of Developmental Disabilities Services – Division of Disability, Aging, and Rehabilitative Services, Medicaid Waiver, and self-pay.
